Metrics/Outcomes Templates

Templates for the most common Board-required tools are in this Excel spreadsheet. This includes the BAM, Ohio Scales, NOMS, and MHSIP. Please check Attachment 4 of your contract to know what to submit and when. For example, if a program listed on your Attachment 4 has the metric "Brief Addiction Monitor (BAM) 30 days after each assessment," that means that 30 days after you conduct a BAM assessment, it should be submitted to the Board using its usual submission process.
